    Responsibilities

    Under the direction of the Director for Innovation & Healthcare Integration, the Lifestyle Coach – Chronic Care Management will provide support and guidance to participants in the YMCA’s Chronic Care Management Programs and participates as part of the extended health care team with local health care providers.  This is a part-time hourly position that may be required to work evenings and weekends to ensure program coverage. A large part of this role will include: Class facilitation, data management and community outreach.

    • Deliver curriculum to class participants in effective, meaningful and compelling ways.
    • Encourage group participation and interaction using open-ended questions.
    • Facilitate commitment and retention of participants.
    • Create a motivating environment that is friendly and non-competitive.
    • Foster relationships with and between participants.
    • Make learning a shared responsibility for the group.
    • Ability to deliver structured presentations and lead group discussions.
    • Provide accessibility to participants both before and after sessions to answer questions and follow-up on any questions you cannot answer during class time.
    • Support and encourage goal setting on a weekly basis.
    • Schedule that allows for flexibility to facilitate Office Hours and Nutrition Education.
    • Record weekly data for each participant Enter data electronically within 48 hours of the class or Office Hours.
    • Ability to provide regular personalized contact with participants for 16 weeks (4 months) to encourage self-monitoring of blood pressure, and attending nutrition education seminars via phone, email or text.
    • Arrive for class, office hours and seminars on time and dressed appropriately.
    • Participate in all Lifestyle Coach Team discussions (meetings, emails or conference calls). Offer ideas and tools for facilitation to the coach team and remain engaged with the team at all times.
    • Attend community events to promote the program and recruit participants.
    • Communicate within and external to the YMCA via online tools and social media to support ongoing growth of the BPSM program.
    • Comply with all HIPAA requirements.

    Qualifications

    • Associates degree from an accredited college or university or the equivalent experience in related field of health, fitness or nutrition.
    • Three years of experience in a related field.
    • One year of experience doing in person community outreach and phone based outreach
    • One year experience using electronic health records
    • Experience preparing, organizing, tracking, and entering data.
    • Intermediate level of proficiency with commonly used office tools such as word processing, spreadsheets, online email and online calendars.
    • Proficient typing and computer skills.
    • Highly detail-oriented, organized, and able to systematically track multiple needs/issues simultaneously.
    • Demonstrated experience and outstanding skills in customer service.
    • Must be able to relate and work with diverse groups of people in a friendly and consistent manner.
    • Schedule that allows for flexibility to facilitate classes
    • Relationship building skills and the ability to build community within a group.
    • Excellent oral and written communication skills.
    • Able to demonstrate high levels of discretion and confidentiality.
    • Able to work effectively with a wide variety of stakeholders (physicians, public health officials, employers, administrators and health seekers)
    • An understanding of behavior change required.
    • Knowledge of basic health, nutrition, and fitness strongly preferred.
    • Knowledge of basics related to chronic disease self-management strongly preferred.
    • Experience with Listen First or motivational interviewing strongly preferred.
    • Must maintain a current motor vehicle license and acceptable driving record that meets YMCA standards.
    • Must currently have and maintain current certification and licensure related to First Aid and CPR.
    • Must be available to work evening and weekend hours.

    Preferred Qualification:
